Practice Partnership Agreement
Section 1: Responsibilities
Your Pets historY LLC will provide a HIPAA-compliant platform for veterinary practices to manage pet health records. Practices agree to ensure all client data is accurate and authorize users appropriately. Both parties commit to maintaining data security and following applicable regulations.
Section 2: Payment Terms
Pricing tiers: All prices shown are introductory rates for the first 6 months. Standard rates apply thereafter as indicated. • Starter: $99/month for up to 50 clients (After 6-month introductory period: $149/month) • Growth: $200/month for up to 150 clients (After 6-month introductory period: $250/month) • Professional: $350/month for up to 400 clients (After 6-month introductory period: $400/month) • Enterprise: $599/month for up to 1,200 clients (After 6-month introductory period: $770/month) Payments are due monthly in advance. Late payments may result in service suspension.
Section 3: Data Restrictions
Partner practices may not share, sell, or distribute pet health data to third parties without explicit owner consent. All data remains the property of pet owners. Your Pets historY maintains compliance with HIPAA, CCPA, and similar regulations.
Section 4: Termination
Either party may terminate this agreement with 30 days written notice. Upon termination, the practice will have 60 days to export all client data before access is revoked. Partial refunds do not apply for early termination within the billing period.
Section 5: Liability
Your Pets historY LLC is not liable for data loss, service interruptions, or indirect damages. The platform is provided "as-is" with no warranty of fitness for any particular purpose. Practices assume all responsibility for clinical decisions based on platform data.
Section 6: Confidentiality
Both parties agree to maintain the confidentiality of proprietary information, trade secrets, and client data. Confidentiality obligations survive termination of this agreement for a period of 3 years.
Section 7: Co-branding & Acceptable Use
Practices may use Your Pets historY branding with prior written approval. Practices agree not to modify, reverse-engineer, or attempt to access the platform infrastructure. Unauthorized use will result in immediate account suspension.
Section 8: Auto Renewal
This agreement automatically renews for successive 12-month periods unless either party provides 60 days written notice of non-renewal. Renewal terms may include price adjustments with 30 days notice.
Section 9: Price Change Notice
Your Pets historY may increase pricing with 30 days written notice to active partners. Partners may cancel within the notice period to maintain existing rates. All price changes take effect at the next billing cycle.
